The GUIDE Program
for Dementia Support

Enhanced dementia care and caregiver support through Medicare. The GUIDE Program is designed to reduce stress, improve care coordination, and provide meaningful support for both patients and caregivers. With access to 72 hours of respite care per year, families can take the time they need to rest and recharge while knowing their loved one is in good hands.

What is The GUIDE Program?

The Guiding an Improved Dementia Experience (GUIDE) Model is a Medicare-supported program focused on delivering comprehensive dementia care. It connects patients and caregivers with a dedicated care team that provides personalized planning, education, and continuous support.

Best of all, your existing Medicare benefits remain unchanged while gaining access to additional services and resources.

Eligibility Requirements

You may qualify for the GUIDE Program if you meet the following criteria:

​

  • Diagnosed with Alzheimer’s disease or another form of dementia

  • Enrolled in Original Medicare Parts A and B

  • Not currently enrolled in Hospice or PACE

  • Not enrolled in Medicare Part C (Medicare Advantage) 

  • Not living in a long-term nursing home
